What does an Affidavit mean in this sentence?

It's something i'm entering in for a contest!

It says "IF THE CHILD IS UNDERAGE THE PARENT WILL HAVE TO FILL OUT AN AFFIDAVIT"

What does it mean?

Just sign a paper when the fedex or ups person comes to deliver?

In this case they must fill out a sworn statement.

It means they want you (the parent) to give your underage child permission to participate. You must sign it and date it.

P.S., about that guy's answer below, you are not going to have to get it notarized....it's a contest, not a tax audit.
